“A 44 year old male, Mr M, with type 1 diabetes presented to A & E with burns to his right foot sustained five days earlier by placing the foot on a hot radiator. The patient was a smoker, had long-term poor glycaemic control and had a number of diabetes associated complications including retinopathy and peripheral neuropathy.”

Treatment of four rare & difficult wounds requiring exceptional wound bed preparation

“Wound bed preparation focuses on debridement, bacterial balance (management of wound exudates) and the overall health status of the patient. Its ultimate aim is to ensure a good granulation tissue leading to complete closure. In this article, we would like to present the wound care regimes for two patients suffering from rare and potentially difficult wounds to treat and heal.” “In each case, the wounds were heavily contaminated with bacteria i.c. MRSA, Enterobacter sp., Proteus sp., loaded with necrotic tissue and required an exceptional level of cleaning and wound bed preparation before further treatment could progress. Cleaning in each case was carried out using chlorhexidine gluconate 0.05% hydrotherapy and a hydro-alginate gel, Flaminal®. In both cases, the wounds were notably cleaner within a week and the wound care regime could be reduced. Wound bed preparation was such that further treatment i.e. skin grafting could be performed after four weeks.

“A clinical pilot study using original methods was undertaken to compare the healing properties of the alginate gel Flaminal® (test) and the hydrocolloid gel Intrasite® (control) on chronic leg ulcers. The study was performed over a period of 28 days with two parallel groups of 10 patients. Both the surface (acetate tracing and planimetry) and the volume (Jeltrate® mould impression and weighting) of each wound were measured at baseline and after 7, 14 and 28 days of treatment. On both parameters results were superior with the test product compared to the control, with volume reduction being the first parameter to change.”

“Flaminal® and Flaminal® Hydro are 2 new hydroactive colloid gel dressings with state antibacterial properties. These properties are attributed to an enzymatic complex in their formulation. In the study described in this report, the antibacterial effects of Flaminal® and Flaminal® Hydro were confirmed in an invitro as well as an in vivo setting. It was also demonstrated that Flaminal and Flaminal Hydro are not toxic to keratinocytes in vitro using an MTT [3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide] test.”
